Transition Coordinator (Hybrid)
Collaborative Courts FSP – Irvine. F/T, Monday - Friday, 9 am - 6 pm. Hybrid schedule available after the training period. Must be available for after-hours emergencies with 24/7 capability to respond via agency issued cell phone.
Responsibilities Include:
- Provide in-home mental health rehabilitation services to youth involved in the Juvenile Justice system.
- Directly assist consumers, ages 12-15, with daily activities related to their Care Plan to become inter-dependent, self-reliant and successful.
- Assist youth in acquiring healthy social skills, positive decision-making skills, school completion, career readiness, independent living skills and conflict resolution skills to reduce recidivism and promote success, safety, wellness, recovery and permanence in the home, school, workforce and community.
- Daily documentation of any service provided.
- Maintain a “whatever it takes” approach to resolving challenges, encouraging and assisting consumers to focus on meeting their goals.
Requirements Include:
- Bachelor’s degree in psychology, social work, criminology or related discipline.
- 1-2 years’ experience in the field.
- Establish rapport and make accurate assessment of a family’s needs.
- Assist families in accessing and utilizing resources provided.
- Excellent written and communication skills.
- Demonstrate the ability to work with a multicultural and diverse population with sensitivity and respect.
- Valid California driver’s license, liability insurance meeting minimum state requirements, and a reliable automobile.

- Bilingual English/Spanish preferred.
Starting pay range: $27.04 - $28.33 per hour
