Transition Coordinator (Youthful Offender Wraparound)

TRANSITION COORDINATOR (Youthful Offender Wraparound - Santa Ana)

– F/T. (Monday-Friday, 9am-6pm, On-call 24/7 for crisis support).

Responsibilities include:

  • Provide in-home mental health rehabilitation services to these youth in order to assist them in acquiring healthy social skills, positive decision-making, school completion, career readiness, independent living skills and conflict resolution skills to reduce recidivism and promote success, safety, and permanence in the home, school, workforce and community.

  • Document services in accordance to Medi-Cal standards including but not limited to same day documentation, 24-48 hour note submission and productivity of 70 DSH per month. Work with Probation, Social Services, Mental Health Staff, Court, educational/vocational partners, family, and other formal and informal resources to teach clients how to access services and builds strong partnerships with community resources.


 

Requirements include:

  • Requires a Bachelors degree in psychology, social work or related discipline.

  • 1-2 years of experience in the field.  Ability to establish rapport, make accurate assessment of the family's needs, assist the family in accessing and utilizing resources, good written and communication skills, demonstrates the ability to work with a diverse multicultural/ethnic population with sensitivity and respect.

  • Valid California driver’s license, auto liability insurance, and a satisfactory driving record.

  • Must be available for on-call coverage for after-hours emergencies with 24/7 capability.

  • Bilingual English/Spanish preferred.
